Select the reason for referral and include the patient's insurance card (front and back), recent office notes, and any relevant prior venous ultrasound reports.

Fax It to Us
Fax the completed referral and supporting documents to 888-464-0609. Our team will review the information and contact the patient to coordinate scheduling.

Conditions We Evaluate
One Vein Clinic provides physician-led evaluations for patients with venous disease and related leg symptoms. Our medical team reviews each patient's history, symptoms, and referral information to determine the appropriate diagnostic testing and treatment recommendations.
When clinically indicated, a venous duplex ultrasound may be performed to evaluate underlying circulation problems.
Available Vein Treatments: Radiofrequency Ablation (RFA), VenaSeal™, Varithena®, and Sclerotherapy.
