3. Forms and communication

When you contact us through the website, please give accurate information. Submitting a form does not book a confirmed appointment until our team contacts you. 4. Text message (SMS) terms

Program: One Vein Clinic patient messages.

What you’ll receive: If you opt in, for example by checking the SMS box on our forms, we may text you about appointments, reminders, follow-up care, and responses to your questions. We send promotional texts only if you have separately agreed to receive them.

• Frequency: Message frequency varies.
• Cost: Message and data rates may apply. Contact your wireless provider about your plan.
• Opt out: Reply STOP at any time. You’ll get one message confirming you’ve been unsubscribed. To join again, opt in the same way you did the first time.
• Help: Reply HELP, call 904-990-0777, or email ovc@oneveinclinic.com.
• Carriers are not liable for delayed or undelivered messages.
• Agreeing to receive texts is not a condition of receiving care.
